We have compiled a list of helpful information regarding benefits for those who have been moved to lay off.
If your employment with BRG has ended, your current coverage will run through the end of the month, based on your employment end date. For example, if your last day is on September 20th, your coverage will be active through September 30th. You will have the opportunity to continue your health plan coverage through COBRA.
What is COBRA?
• COBRA stands for Consolidated Omnibus BudgetReconciliation Act. This is a federal law that gives you the option to continue your current health insurance coverage for up to 18 months. You're able to stay on your former employer's plan for a limited time. It's the same plan and you get the same care and benefits
• To qualify, you must have been employed and covered under the BRG health plan and experienced a job loss due to termination, layoff, resignation, or retirement. Please note that 100% of COBRA expenses are covered by the individual.
How does COBRA work?
• Allied Benefit Systems will provide you with aCOBRA packet both via email and US mail – please ensure that BRG has your updated email and mailing address to ensure you receive this information. This packet will provide you with details on how to continue coverage through COBRA and how to provide payment.
• You will have up to 60 days to decide whether you want to continue health care coverage via COBRA. If you elect to continue under COBRA, your coverage will start the day after your plan coverage through BRG ends.
• COBRA coverage will terminate early if you do not pay your premiums or if you find new employment that offers health insurance coverage before COBRA runs out.

You are able to continue your Vision plan coverage via COBRA. EyeMed will send you a COBRA packet with details on how you can continue coverage and how to submit payment. If you have not received your COBRA packet from EyeMed, please reach out to hr@bokagrp.com.

If you are enrolled in Accident, CriticalIllness, or Hospital Indemnity benefits, you have the option to take or “port” these benefits with you and keep the same rates. The Standard team will reach out via US mail with all the details on how you can port your coverage and send in your monthly payment.
• Standard Voluntary Benefits - How to Continue Benefits
If you are enrolled in Life Insurance your plan can be ported or converted based on the requirements below.
• Porting coverage for Whole Life insurance – you are eligible to port your life insurance if you are under the age of 75, have been covered under the life insurance plan for at least 12 months and you are not sick/injured.
• If you do not meet the above requirements, you will be able to convert your life insurance into an individual whole life coverage. Please note that conversion of life insurance will be more expensive than the original group term life insurance coverage you enrolled for through BRG.

What if I choose not to continue coverage through COBRA or I was never covered through BRG, but still need health insurance?

If the form is completed and returned within 90 days of receipt, the distribution willbe made according to your election choice.
If you do not return the form or contact John Hancock within 90 days,
• if your total vested balance is $1,000 or less your funds will be paid to you in a lump sum cash distribution.
-OR-
• If your total vested balance is over $1,000 it will be rolled over into an IRA established by John Hancock. The IRA will impose maintenance fees which will be deducted from your IRA. You may transfer the IRA funds at any time to another IRA or retirement plan that will accept such amounts.

What if I choose not to continue coverage through COBRA or I was never covered through BRG but still need health insurance?
• Benefit counselors from BRG’s insurance team are available to assist with exploring alternative options for health insurance coverage. Please reach out to hr@bokagrp.com for further assistance getting in touch with our insurance team.
• Visit Illinoi's Marketplace getcovered.illinois.gov/en: provides additional options for health care coverage through the marketplace. Here you can find information on subsidizing the cost of insurance premiums and low-cost options for health insurance coverage for children. Local counselors are also available by appointment if you would like one-on-one assistance reviewing coverage options as well.
